Thank you for bringing your concerns to our attention. We understand how frustrating it can be when you are waiting for an important document, such as a birth certificate.

After reviewing Order VRO1978475, we can confirm that your application is currently on hold because the notarized form submitted is missing the date on which it was signed. Unfortunately, the issuing agency requires this information in order for the application to be considered valid. Without a complete notarized document, we are unable to submit the request to the Health Department for processing.

Once your payment was completed, you received an email with a link where additional information and the required documents needed to be uploaded in order to finalize your application before it could be sent to the issuing agency. Our records show that the application remains incomplete, which is why the status continues to appear as “under review.” Since your order was placed, several emails have been sent requesting that the missing information be provided so we can proceed with submitting the request.

Once logged in, you can access the Support/Application section and upload the corrected notarized document and any remaining information needed. As soon as the required details are completed, we will be able to move forward and submit your request to the Health Department.
